Understanding  between Student and Parent/Guardian and MSCD governing  student      participation in  community work-based learning (aka “offsites”) with particular focus 

upon active student participation during in-person, blended, and/or remote instruction.

I consent to participation in the above-named community work-based learning site. I understand that  during the period of Covid-19  restrictions, the instruction may be in-person at the MSCD main-site, blended learning, or remote learning, or a combination of all three instruction methods.


I agree that the student is to behave responsibly during instruction and to follow the school’s discipline code and policies regardless if the instruction is provided in-person, through a blended model, or remotely.


I agree that during instruction, either in-person, blended, or remote, students will evidence respect for time by being punctual in reporting to class either physically or logging-on; respect for supervisors by using proper language and behavior as well as following directions; respect for safety by working in a safe manner; respect for tools by using and maintaining tools properly; respect for co-workers by helping others when needed and assisting in site activities..


I understand that student evaluations and grades are  derived from a combination of student performance during the academic and work experience portions of the day.  A “passing” grade will require students to demonstrate target behaviors with cues from the teacher. An “exemplary” grade will require students to demonstrate initiative with minimal supervision required of the teacher.


Regardless if instruction is delivered in-person, in a blended fashion, or remotely, the below-signed student and parent/guardian commit themselves to  actively facilitate student’s academic and social success by strict compliance with this agreement.
